Foster Care Case Manager


Job Summary:

Case management responsibilities for certified resource families, prospective resource families, and children assigned for case management


Essential Functions of the Position:

  1. Provide case management for children in Bethesda resource homes to include coordination of Individual Service Plan (ISP) meetings, Progress Reports, gather necessary documents as required, and conduct semi-annual updates to the ISP.
  2. Conduct home visits and maintain phone contact with assigned resource families pursuant to department policies and procedures.
  3. Maintain contact with children assigned for case management pursuant to contractual guidelines.
  4. Maintain logs of all contacts with children and families assigned.
  5. Participate actively in identifying resource homes for children referred and communicates with placing agencies to insure optimal services to children and foster families.
  6. Serve as a liaison between resource parents, placing agencies, and other services providers.
  7. Meet with supervisor one-one-one for supervision biweekly.
  8. Maintain annual in-service training hours as required by personnel policy and attend additional training for professional development.
  9. Adhere to federal, state, local, and agency regulations regarding confidentiality.
  10. Provide on-call to resource families on a rotating basis with the resource staff team.
  11. Perform other related duties as assigned by supervisor.


Required Education, Experience, and Clearances:

  1. Requires a Bachelor’s Degree in a human services field.
  2. Requires one year experience in working with children and families.


Salary:

$18-$20 per hour
