Foster Care Case Manager
Job Summary:
Case management responsibilities for certified resource families, prospective resource families, and children assigned for case management
Essential Functions of the Position:
- Provide case management for children in Bethesda resource homes to include coordination of Individual Service Plan (ISP) meetings, Progress Reports, gather necessary documents as required, and conduct semi-annual updates to the ISP.
- Conduct home visits and maintain phone contact with assigned resource families pursuant to department policies and procedures.
- Maintain contact with children assigned for case management pursuant to contractual guidelines.
- Maintain logs of all contacts with children and families assigned.
- Participate actively in identifying resource homes for children referred and communicates with placing agencies to insure optimal services to children and foster families.
- Serve as a liaison between resource parents, placing agencies, and other services providers.
- Meet with supervisor one-one-one for supervision biweekly.
- Maintain annual in-service training hours as required by personnel policy and attend additional training for professional development.
- Adhere to federal, state, local, and agency regulations regarding confidentiality.
- Provide on-call to resource families on a rotating basis with the resource staff team.
- Perform other related duties as assigned by supervisor.
Required Education, Experience, and Clearances:
- Requires a Bachelor’s Degree in a human services field.
- Requires one year experience in working with children and families.
Salary:
$18-$20 per hour
