Foster Care & Permanency Coordinator 


Job Summary:

Responsible for the coordination of permanency services. This position will seek to ensure trauma-informed care and the creation and maintenance of a trauma-informed environment for all youth in the program.


Essential Functions of the Position:

  1. Coordinate Statewide Adoption Network services within the contract and ensure benchmarks are met.
  2. Serve as a liaison between resource caregivers, placing agencies, and other service providers.
  3. Assist with the training of newly hired Permanency Specialists and Resource Care Case Managers.
  4. Ensure that resource family files are in compliance with state and federal mandates.
  5. Attend meetings, conferences, and workshops related to resource care and/or permanency services.
  6. Assist with the coordination and delivery of the on-call system of resource care program.
  7. Maintain caseload and timely completion of documentation required in recordkeeping systems (SWAN portal/myEvolv).
  8. Actively participate in agency team meetings and committees.
  9. Assist in the coordination of placement and case management services for children referred for resource care.
  10. Conduct home studies to certify and/or maintain certification of resource families pursuant to licensing requirements.
  11. Provide in-home supervision and supportive services to resource caregivers.
  12. Assist Resource Care Case Managers with case management responsibilities, as needed.


Required Education, Experience, and Clearances:

  1. Bachelor degree in the human services or related field required. Master’s degree in the human services or related field preferred.
  2. Knowledge and experience in the child welfare system, foster care, and/or SWAN services with at least two years of supervisory experience.
  3. Must be at least 21 years of age.
  4. FBI, Child Abuse History, PA State Police, and Driver Clearances. Other clearances may be required dependent on residency status.


Salary:

$20-$22 per hour
